Is Pelvic Physical Therapy Only For Women?

A phrase often heard in our waiting rooms is, “I’m so glad I’m not the only male waiting here.” Believe it or not, not just those who were born female have a pelvic floor. Not only those who are pregnant or postpartum need pelvic PT. Those who have penises have a pelvic floor, and they can also have pelvic floor dysfunction. Diagnosis that we commonly see in the clinic, but are not limited to:

  • Nonbacterial prostatitis

  • Interstitial cystitis

  • Constipation

  • Erectile dysfunction

  • Pain associated with intercourse (before, during, or afterwards)

Fortunately, pelvic physical therapy has been shown to help with the rehabilitation of male pelvic dysfunction.

Signs and Symptoms of Male Pelvic Dysfunction

Urinary urge and/or frequency

  • Difficulty eliminating urine

  • Urinary incontinence

  • Difficulty eliminating stool

  • Fecal incontinence

  • Pain in the testicles and/or scrotum

  • Penile pain

Evaluation

Just like all other evaluations, we first start the evaluation with a conversation inquiring about what is going on. We ask about your symptoms (bowel, bladder, sexual), we take into consideration your medical history, surgical history, if you have had an active infection, and much more. 

After the conversation, we move towards the examination. We start with everything outside of the pelvic floor, and we take the whole body into consideration. We are trained to be able to assess if anything outside of the pelvic floor can be contributing to your pelvic dysfunction. We look at your musculature, movement patterns, nerve involvement, connective tissue involvement, amongst other things.

Following an external examination, we look at the pelvic floor muscles. This means your therapist will use a single gloved finger, and assess the pelvic floor muscles through the rectum. This is always done with informed consent. Here we are assessing how well the muscles contract, relax, and lengthen. Further, we are looking at the strength of the muscles, tension, range of motion, and how well you can control them. 

At the end, your therapist will put everything together and come up with a plan on how to get you back doing the things you love.

For example, if your body needs to decrease muscle tension, your plan will include treatments to do so. Vice versa, if your body needs strengthening, then your plan will include just that.
